We accept referrals for:

  • General psychiatric evaluations.
  • Bariatric surgery psychiatric evaluations.
  • Gender affirming surgery psychiatric evaluations, and gender affirming care.
  • Psychiatric medication management.
  • Psychotherapy in OR & WA.
  • Medication management for opioid use disorder. 
  • Telehealth available in all states. In-person visits for psychiatric medication management and evaluations also available in in Clackamas & Portland, OR; Issaquah, WA; and Salt Lake City, UT. In-person visits for psychotherapy also available in Portland, OR. 

Financial:

  • OR, WA, MN, UT, AZ, NM, NV, MA: See in-network insurances and discounted self-pay rates here.

Patient Criteria:

  • Reside and located in Oregon, Washington, Utah, Minnesota, Arizona, New Mexico, Nevada, or Massachusetts.
  • Ages 18+ in all states. 5+ in OR, WA, MA; 8+ in AZ; 13+ in UT, NV; 16+ in NM; and 18+ in MN for medication management. 12+ in OR and 15+ in WA for psychotherapy.
  • Appropriate for telehealth level of care (contact us at admin@skycloudhealth.com with specific questions about these criteria).
  • We prescribe controlled substances only in Oregon, Washington, Utah, and New Mexico. In-office visit is required.
  • We offer opioid use disorder treatment in Oregon, Washington, and Utah.
  • We do not offer evaluations for disability or legal reasons.
