We accept referrals for:

  • General psychiatric evaluations for adults and adolescents.
  • Bariatric surgery psychiatric evaluations.
  • Gender affirming surgery psychiatric evaluations.
  • Psychiatric medication management.
  • Psychotherapy (limited availability at this time).
  • Medication management for opioid use disorder. 

Financial:

  • OR, WA, MN, UT, AZ, NM, NV: Commercial insurance plans, Medicare, and self-pay.  
  • OR: Accepting CareOregon and Open Card OHP. 
  • NV: In process of contracting with Nevada Medicaid.

Patient Criteria:

  • Reside and located in Oregon, Washington, Utah, Minnesota, Arizona, New Mexico, or Nevada.
  • Ages 18+ in all states. Ages 13-17 in OR, WA, UT, NM, AZ, and NV.
  • Appropriate for telehealth level of care (contact us at admin@skycloudhealth.com with specific questions about these criteria).
  • We prescribe controlled substances only in Oregon, Washington, Utah, and New Mexico. In-office visit is required.
  • We offer opioid use disorder treatment in Oregon, Washington, and Utah.
  • We do not offer evaluations for disability or legal reasons.
