Discounted Self-Pay Rates
Initial Psychiatric Evaluation: $395
Length of appointment: 60 minutes
Description: This is your first appointment with one of our providers at Skycloud Mental Health. During this appointment, information will be gathered about the nature and history of your symptoms as well as information about any past psychiatric treatment. A diagnosis will be determined and we will develop a treatment plan. Medications can be provided at this first appointment if determined to be appropriate. If you are able to, please provide a copy of your most recent lab work and blood pressure.
Follow Up Appointments (Medication Management):
Length of Appointment: up to 30 minutes $195, up to 60 minutes $350
Description: Once your symptoms have been adequately managed and treated with medication management, care will continue to be provided through medication checks to ensure ongoing mental wellness. Each additional 15 minutes beyond first half hour are charged $87.50.
Form completion: $85
Discounted self-pay rates are due entirely at the time the appointment is booked. Full refund can be made for cancellations up to 24 hours in advance of appointment time. Less than 24 hours notice or appointment no-shows will receive no refund. Late cancel/no-show fee is up to entire self-pay appointment rate.

Please note that these rates are billed to your insurance company and you then owe a balance based on your specific plan’s allowable amount, copay, coinsurance, and/or deductible when using in-network coverage to pay for services. The appointment charge may be a combination of two codes, depending on the services rendered during the appointment. Please email billing@skycloudhealth.com for an estimation of your coverage and benefits and allow 1-2 business days for a response. Note that we do not guarantee coverage or benefits information and it is your responsibility to verify directly by calling the Member Services number on the back of your insurance card.
Skycloud Mental Health requires a credit card on file for all new patients. Note regarding credit cards on file for insurance patients: When a balance is deemed your responsibility by our practice (coinsurance, deductible, etc.), you will receive a notification by email when the charge/detailed summary of charge is available. The email will give you an opportunity to review the charges in your patient portal. Your card will be charged ten days later.
