An in-office visit is required once a year in Clackamas OR, Issaquah WA, Layton UT, Lehi UT, and Albuquerque NM for certain medications. Medications are prescribed only after a thorough evaluation at the discretion of your provider. Please thoroughly read our controlled substance policy. While we prescribe psychiatric medications in Arizona, Utah, Oregon, New Mexico, Washington, Nevada, and Minnesota, we only prescribe controlled substance medication when clinically appropriate to patients in Oregon, Washington, Utah, and New Mexico.
