Some plans have a limit on the number of outpatient therapy sessions that are covered by the insurance company, usually around 30 sessions. There are generally co-pays or coinsurance requirements for each visit.

Co-pays/Coinsurance
With Providence, some plans have a $5 co-pay for your first three behavioral health outpatient visits. After that, your co-pay will increase to $30 per visit for an in-person therapy session or $10 per visit for a virtual therapy session.
These are, however, average costs, and the exact requirement for your co-pay or coinsurance payment is based on your plan.
Prior Authorization

Providence Therapy Coverage for Inpatient Treatment
Providence covers inpatient therapy services, but with slightly more limits, such as a limit of 30 days for an inpatient program. If you are looking for Providence addiction treatment coverage, you might have more severe limits on an inpatient program but more flexible limits for an outpatient program.
With an inpatient program, you may also be expected to meet deductible requirements and coinsurance requirements before the insurance provider will cover the cost of your treatment.

- Detoxification with or without medication
- Inpatient care
- Outpatient programs

- Bronze: The bronze plan is the lowest in terms of your monthly premium but the highest in terms of out-of-pocket co-pays and deductibles
- Silver: The silver plan is somewhere in between in terms of monthly premiums as well as out-of-pocket expenses
- Gold: The gold plan has the highest monthly premiums, but in exchange, you get the lowest deductibles and co-pays
