Hi! I’m Madeline Jacobs, a board-certified Psychiatric Mental Health Nurse Practitioner. I hold a Masters in Nursing from Oregon Health & Science University and have worked in the psychiatric field since 2015. I’ve worked at the Los Angeles LGBT Center and would love the opportunity to broaden the reach of LGBTQIA+ affirming mental health care through telepsychiatry. I take a trauma-informed perspective on mental health care, specializing in mood disorders and transgender-related mental healthcare.

We are accepting self-pay patients for psychotherapy in Oregon and Utah. Our provider is in process of credentialing with several insurances. We have a waitlist for clients in Washington.

We are accepting PacificSource, Aetna and self-pay patients for psychotherapy in Oregon and Utah. Our provider, Nat Day LCSW, is also in process of credentialing with other insurance panels - Cigna, BCBS, and Moda. Our psychotherapy waitlist is open for patients located in Oregon, Washington, and Utah with Cigna, Aetna, PacificSource, United, Moda, and self-pay pay sources only. The waitlist is currently closed for Regence (BCBS) and Providence insurance. Our psychotherapy providers treat ages 18-64.
